We describe the
CT and MRI findings of a rare case that had persistence of multiple emissary veins and presented clinically with tinnitus.
The radiological findings included a dilated left mastoid emissary vein, bilateral petrosquamosal sinuses, posterior condylar
veins, occipital emissary veins and an intrapetrous venule. The left petrosquamosal sinus had an unusual origin from the dilated
mastoid emissary vein. The patient also had major anomalies of posterior fossa venous sinuses which are discussed. A relevant
review of literature is included.

Phantom perception refers to the conscious awareness of a percept in the absence of an external stimulus. On the basis of basic neuroscience on perception and clinical research in phantom pain and phantom sound, we propose a working model for their origin. Sensory deafferentation results in high-frequency, gamma band, synchronized neuronal activity in the sensory cortex. This activity becomes a conscious percept only if it is connected to larger coactivated â(self-)awarenessâ and âsalienceâ brain networks. Case history A 33-year-old man presented with sudden-onset unsteadiness, oscillopsia, nausea and vomiting, and left ear tinnitus and hearing loss. One month prior, he had developed lethargy and epigastric and thoracic back pain. Liver function tests were abnormal with a mixed hepatitic and obstructive picture. The nausea and vomiting settled, but 5 days after the development of the vestibulocochlear symptoms, he woke up with painful red eyes which resolved within 24 h. Over the following 2 weeks, the patient noticed a deterioration in hearing with probable complete loss in the left ear, a reduction in the right and bilateral tinnitus. Tinnitus has been associated with increased spontaneous and evoked activity, increased neural synchrony, and reorganization of tonotopic maps of auditory nuclei.
